An innovative program involving ‘social prescriptions’ to help people whose mental health significantly impacts their daily living.
NDIS Services
As an Approved NDIS Provider, we offer services for complex health issues, long-term mental health conditions and psychosocial disability.
GP Social Work Connect
Assisting GPs to address the psychosocial needs of their patients, liaising with welfare providers and arranging linkage to appropriate supports.
Psychological Access +
Psychological Access+ is primarily for people who have difficulty accessing appropriate mental health support due to financial or other circumstances.
Plus Social for Injured Workers
Plus Social is a program to link injured workers to support services and community groups to complement their medical recovery.
Well Together Groups
Our Well Together groups are specialised groups providing supported social participation to build wellbeing, inclusion and resilience.
About Us
Primary & Community Care Services (PCCS) is a local non-government, not-for-profit organisation that works to make it easier for people to access the health and community services they need, and make the journey smoother for those who deliver these services. PCCS provides a range of programs that deliver nursing, occupational therapy, social work and psychological support services in the community. Our strengths lie in care coordination, linking and providing support services.
