Our Mental Health Triage Team

Our Mental Health Triage team (phone (07) 3186 4000) provides a central point for receiving and assessing referrals for people who are not in crisis and require mental health, drug and alcohol and suicide prevention services.

Our multi-disciplinary team will coordinate intake for the Gold Coast Primary Health Network’s mental health care services. In a stepped care approach, a person presenting to the mental health system is matched to the intervention level that most suits their current needs.

PCCS’ welcoming team is well-placed to identify a person’s needs and ensure referral to the correct service. By triaging all referrals for the Gold Coast region, our aim is to ensure no person in need falls through the cracks and enables people from underserviced groups to access varying levels and intensity of care determined by their needs.

Plus Social – Eligibility requirements

Plus Social is free to Gold Coast (Queensland) area residents who are:

  • Are 18+ years old and living with complex mental health issues
  • Living in the community in the Gold Coast area and supported by a GP or private psychiatrist
  • Would benefit from increased care coordination and links to supports
  • Would like increased social supports or to access social groups
  • Are in need of (non-urgent) additional support beyond GP management, and not receiving supports from Gold Coast Health or Queensland Health.

The program is only accessible through a referral from a GP or psychiatrist. The referral form is available here.
