Non-medical support for your wellbeing

Social Rx^® is a short-term program designed to link you to locally available support services and community based resources using a social prescribing approach.

PCCS Australias-leader-logo PCCS can help you to tackle the non-medical barriers to your health and wellbeing, issues such as housing, social and community networks, family relationships and other socioeconomic factors that you and your doctor cannot address alone. We work with you to identify meaningful goals and to develop a personalised plan to meet these needs while reducing isolation and disadvantage.

How we help

Social Rx is designed to address gaps between health and social systems, providing practical supports and linking people to the community resources and supports they need, working together to get the best outcome possible.

Our team of social workers can assist with things like:

Social Programs & Services like art classes, book clubs, coffee clubs, knitting groups, and community centres
Physical Activity Programs like walking groups, chair yoga, lawn bowling, Healthy Weight for Life (including online groups people can do at home)
Support to Access Government Services like like My Aged Care, NSW Housing, Centrelink, and linkages for assistance with NDIS applications and for Chronic Care Coordination
Healthy Lifestyle/Food Programs like Meals on Wheels, food banks and cooking groups.

Locations

Central & Eastern Sydney, NSW

Social Rx is provided at no cost to anyone over 18 years living or working or studying in the Central and Eastern NSW PHN area (see the shaded area in the map below). More specifically, it is intended for people who:

have or are at risk of developing, a long term health condition (including mental health), or
are experiencing social isolation or loneliness, or
need practical help with issues that could significantly impact on their health and wellbeing like housing issues, food security, lack of access to aged care or disability supports, domestic violence, financial stresses, low physical activity or any other significant stressors.

*Social Rx is not intended for people who already have access to other long term supports like the NDIS, psychosocial supports or are living in a RACF.

Talk to our friendly team for more information and to discuss access to the program. You can also ask your GP about this program and for a referral (you could direct them to this page).

South Eastern NSW

Social Rx is provided at no cost to anyone over 18 years living in South Eastern NSW PHN area (stretching from Helensburgh in the north, past Eden to the Victorian border in the south, and inland to Cooma/Monaro, Queanbeyan, Yass and Goulburn). More specifically, it is intended for people who:

have or are at risk of developing, a long term health condition (including mental health), or
are experiencing social isolation or loneliness, or
need practical help with issues that could significantly impact on their health and wellbeing like housing issues, food security, lack of access to aged care or disability supports, domestic violence, financial stresses, low physical activity or any other significant stressors.

*Social Rx is not intended for people who already have access to other long term supports like the NDIS, psychosocial supports or are living in a RACF.

Gold Coast, QLD

Social Rx is provided at no cost to anyone over 18 years living in the Gold Coast PHN area (see the dark blue shaded area in the map below). More specifically, it is intended for people who:

have or are at risk of developing, a long term health condition (including mental health), or
are experiencing social isolation or loneliness, or
need practical help with issues that could significantly impact on their health and wellbeing like housing issues, food security, lack of access to aged care or disability supports, domestic violence, financial stresses, low physical activity or any other significant stressors.

*Social Rx is not intended for people who already have access to other long term supports like the NDIS, psychosocial supports or are living in a RACF.
